In order to apply for a moving van permit you must provide a photo ID and a proof of address. You can apply in person at City Hall Room 209 or online at chelseama.gov .You must allow for a minimum of 10 business days for the application to process. 

Proof of Address may be a utility bill, cellphone bill, hospital bill, credit card bill, cable bill no older than 60 days. If you are a new resident and are moving in you may provide a copy of your lease or purchasing agreement as a proof of address. The fee is 50 dollars per space.
